Frequent Rapid Heartbeats: Understanding the Cause Early

成人的正常心跳速率通常维持在每分钟60至100次之间。当你突然感觉到心跳加快且伴随着身体的不适症状时,这些健康警示可能表明你需要及时就医以厘清病因。 萨特温德医生表示,现代人们普遍都穿戴智能手表,它不仅是一个时尚配件,人们可以善用它的功能帮助自己监督健康状况。如果观察心跳过加快应及时就诊。 “心脏在每一刻都不停地工作,在正常情况下,成人若静止不动,每分钟的心跳次数约60至100下。若没有从事任何活动,心跳也频频超过100下以上,便可能是心波过速,也就是心跳过快。” 除了心跳过快,患者也可能会感到头晕、昏厥,虚弱、心悸、四肢无力或其他不适的症状。临床上将心律不整分为两大类,即心跳过快或心跳过慢,再考虑心律规则性,用以诊断心律不整的种类。 心律不整的种类很多,临床上最常见的是心房颤动,另外还有上心室心搏过速(心室上频脉)、心房扑动、心房频脉,心室性心博过速(心室频脉)和心脏传导障碍等。 通常医生诊断有心跳过快问题的忠者,他们需佩跋动态心电图监测仪附于胸壁的电极感知和记录心胜电活动,可连续监测24至48小时。医生从心电记录观察是否任何不规则的心 律,若有心跳过快者需要进行电生理学研究程序。 电生理学研充(Electrophysiological Study,简称EPS)是一项用于评估心脏电系统,并检查心律异常的测试。医生将细小的电极导管插入通向心脏的血管中,电极导管引向心脏的4个特定位置,电极导管的另一端则连接着电脑,以测量心脏的电信号。电信号也通过电极发送来刺激心脏组织,试图引起心律异常。这样做是为了对其进行评估并找到其原因。 电生理学研究中的即时心脏消融术 在电生理学研究期间,医生可以获取与异常快或慢心律的相关资讯,一旦找到心律问题的异常区城后,医生将采取心脏消融术(ablation)。换言之,医生在电生理学研究期间也同时进行心脏消融术,彻底为患者解决病灶。 萨特温德医生解释,心心脏消融术是目前快速治疗心律失常的首选方法,主要分为导管顶端输送热能(频物)或冷能(冷冻消融),大约90%的案例都是采用热能消融术。亚松大医院也在近来几个月成功完成多宗消融手术,成果令人鼓舞。 医生会透过导管经血管进人心胜以释放微量热能或冷冻能量。当导管放到引致异常心律的位置后,释出的能量可形成微小心脏组织的损害,使其失去制造电流的功能或阻止不正常的电脉,借以治愈心律失常。完成消融术将撤出导管,手术过程约15至40分钟。 他强调,这些损害只会摧毁与错误心脏模式有关的组织,不会引起任何残余疼痛或对心脏功能造成问题。 改善生活质量 萨特温德医生指出,部分患者经历心跳过快的情况,不取鼓起勇气就医,将会对生活及健康造成持续影响。其实心脏消融术以非手术的侵入性方法,达到病灶局部进行区域性浴疗;伤口较小,患者通常在心导管手术后的第二天便可以下床甚至出院。 基于上述优势,即使是年纪较大的患者、孕妇或正在哺乳的母亲,均能够接受消融手术,从而有助于提高患者的生活质量。不论是进行电生理学研究还是消融手术,患者在此之前需要进行血液检查,至少需要然禁食4小时,并需取下身上的金属饰物。 除了热能或冷能消融术,随着医疗科技的快速发展,最新的3D心电生理检查也为患者造福。它提供了动态而立体的影像,通过使用电磁技术生成患者心脏结构的3D影像。导管头端与心脏的3D立体影像能够即时显示在电脑荧幕上,甚至能够准确呈现导管头端的位置和指向。 医生只需注视电脑荧幕,就能清晰了解导管头在心脏内的位置、指向以及与心内壁的接触情况。通过观察电脑荧幕上导管的移动,医生能够更加淮确地判断病变位置。这项技术使得医生能够更精准地引导导管至心脏中需要进行治疗的确切位置和方向,犹如使用导航器一样。 🌐︎ www.assunta.com.my 图说1: 萨特温德医生 (Dr Sathvinder Singh A/L Gian Singh) 亚松大医院心脏和电生理学专科 图说2: 正常的心电图 VS 心房颤动 —
